Disappearing Avios availability during the booking process

Just made an Avios reservation to Portugal, and on the return leg there were 5 seats available in Club Europe.
There are 5 of us (2 adults, 3 kids) and I had 2 x companion vouchers to use up, so I decided to book for myself+3 kids, and then separately for my wife (so as to choose the 50p tax option for her, which isn’t available on the companion voucher).
After booking for the 4 of us, the single remaining Avios seat remained available for my wife – but upon reaching the end of the process (after payment) I was informed that it had disappeared, and hence her booking didn’t go through.
Has anyone experienced this? and is the seat likely to pop up later (no other flights that day)? or was it likely a “phantom” seat? I think it very unlikely it was booked up by someone else at that exact moment…
In hindsight perhaps I should have just attempted to book all 5 together (and also get better value for the Avios…) but it is what it is…
@jsto I have seen this happen quite often and then seen the seat come back. It happened to us last week – booked 2 x F seats with a 241, when three were showing and I wanted the third for my son, but had to make on a different booking as he is travelling out on a different day. Third seat disappeared, but like you, convinced that stray seat hadn’t been taken and it duly reappeared two days later after SeatSpy alert set.
