yes, but in these circumstances I can imagine one would just book a cash flight, and claim that, if the original booking was in miles.
cover for the very narrow possibility you have a sudden illness in the 23h59m before first departure is optional in my book, and I’m a stickler for traveling with insurance. everything else can be rearranged with cash more easily, surely? or am I missing something?
