Is it possible to cancel an avios one way flight for a fee and get avios back in account? I seem to recall this is possible, and how soon before departure does it need to be?
Got a potential medical issue that might prevent me flying next week to USA

You should be able to do this right up to the day of travel via MMB (look for “cancellation options”). You’ll get a message saying you’ve applied for a voucher but you will actually get a full refund of everything because it’s within the BWC date.
Rather than going via MMB you can apply for a FTV directly via this link

As it is for Avios BA will not give you a FTV but will refund the Avios into your BAEC account.
And there is no fee right now due to book with confidence
