Closing card account with a balance

If you want to close a card but you still have a balance, do you have to pay off the balance immediately or will Amex send you a statement post-closure that will get paid by direct debit if you have it set up?

Yes, you just pay off the balance in the normal way.
On some cards the unused part of the annual charge will be refunded, so make sure that you owe at least that much for ease.
