Past comments here indicate it’s OK for just one person to return on a 2-1.
We’re in South Africa and I can have a medical procedure which means not flying for a few days. My wife must get home on the 2-1 voucher, I can make my own way back OK later on a different ticket.
We don’t want any issue with my wife being denied boarding if I’m not there.
Has anybody had a problem with this in the past, and do the experts here think I should check in and not turn up, or just check my wife in?

Is the 2-4-1 voucher in your name or your wife’s? If it’s in your name your wife may have a problem flying on her own as the lead passenger must accompany her (per the T&Cs).
I think the no shows where it’s not the lead passenger is OK. Happy to be corrected on this.
Might be worth a call to BA and see if you can amend your return date separately due to the medical procedure you wish to undergo but I’d be prepared for a negative response from BA.
I didn’t have to do this, but a few years ago had a booking for four, with me as lead traveller, two companion vouchers in play, and there was a risk I’d not make the return flight.
I was told it was OK if I no-showed for the return (absolutely not ok for the outbound I suspect!). However, I didn’t need to put it to the test.
Thinking logically about this there a dozens of reasons why one traveller is unable to fly or needs to return early — BA are hardly likely to want the Daily Mail headlines about a stranded family the strict enforcement of the voucher terms would entail.
Thanks, I am the lead traveller but sounds like it’s OK. I think if I decide to remain, I’ll check in as normal but not turn up at the counter.
If they make an issue of it, I’ll have to decide whether to travel or not at the time, tricky!
