Looking to fly to Milan using Avios on a one way ticket. Cash fare same flight £242 each.
    Avios options

    32500 plus £25 = £350 or £175 each @ 1p per Avios
    11500 plus £159 = £279 or £137 each @ 1p per Avios

The 2nd option seems best value but am I doing this right?

The first option is probably what I will go for as travel is in doubt and while happy to lose £25 not so keen to risk £70.

    Isn’t the refund 50p?

The penalty for cancelling is the same whichever option you choose. i would take the second; as you say, it’s better value. You save 21,000 Avios for £144, effectively buying them for 0.69p each.

This is a fantastic example of the outsize value that can be delivered by the programme, despite the frequent moans we read here about devaluations. Taking the second option, you save £325 for two people at the cost of 11,500 Avios – a return of 2.8p per Avios. Absolutely massive.

Reports from last week that cancellation fee is still 50p.
I agree with @jj about the overall value.
I’m also seeing a 7,250 points plus £59.50 option which is a touch cheaper.

I thought that the cancellation of an Avios ticket was either £35 per person or the total amount if that was less. What have I missed?
